Kevin Yohannan, DO

 Dr. Yohannan is a board-certified Family Medicine Physician who completed his Family Medicine Residency training at Garnet Health Medical Center in Middletown, NY. He further went on to complete his fellowship training in Hospice & Palliative Care at Northwell Health - North Shore University Hospital in Manhasset, NY.

In his fellowship training, he developed a steadfast commitment to improving the quality of life for patients with life-limiting and serious illnesses. He gained experience across multiple care settings in an Age-Friendly Health System including managing a Palliative Care Unit, acting as a consultant on an inpatient service, and continuity of care in the outpatient care setting.

He collaborated closely with Oncologists, Intensivists, Chaplaincy, Social Work, and other medical specialists to help provide an interdisciplinary approach to patient care. He is experienced in conveying complex medical information with contextual, cultural, and religious awareness, building trust with patients and their families to navigate decision making.

Dr. Yohannan is skilled in ensuring patients have a seamless transition between care settings including the hospital, home or hospice care. He is experienced in working with a diverse patient population and identifying what matters most to the patient in order to tailor their care accordingly.

Throughout my training, I gained experience with a wide range of medical complexities, including managing oncological malignancies, neurodegenerative conditions, organ transplant evaluations, and the withdrawal of life-sustaining care. I am well-versed in addressing complex pain, ensuring effective symptom control, and guiding conversations around end-of-life care with compassion and clarity.

What is the most important thing your patients should expect from their professional relationship with you? What do you expect from them?
Patients can expect transparency, honesty, and understanding. I strive to foster a space for patients to feel comfortable, valued and respected. I hope that my patients see that they are more than just patients, but that they are a unique individual who deserves care that adds meaningful life to their days.

How do you spend your valuable personal time when you are not caring for patients?
I enjoy making pastries and spending quality time with my family. I am also a car enthusiast!

Why did you choose your specialty and what is its greatest reward?
Hospice & Palliative Care provides an opportunity to allow a patient’s voice to be heard and that can not only change the entire course of their medical care, but also their life. In my perspective, the greatest reward is when a patient realizes how powerful their voice truly is and feeling dignified throughout their medical journey.
